Vital signs assessment involves measuring the body’s basic functions. Nurses routinely check temperature, pulse, respirations, and blood pressure. Normal vital sign ranges exist, but each patient’s baseline can differ. Tracking vital signs provides objective data about the patient’s status and response to care, so nurses should assess vitals regularly and recognize changes that may indicate patient deterioration.
